Breathlessness is one of the most common and distressing symptoms experienced by people living with interstitial lung disease (ILD). For many, it is the symptom that first prompts medical attention. For others, it develops gradually and becomes more noticeable with exertion. Understanding why breathlessness occurs, and what can be done about it, is often reassuring. Although ILD involves scarring of the lungs, the sensation of breathlessness is influenced by several overlapping mechanisms. It is not determined by a single scan result or lung function number.
Why does ILD cause breathlessness?
In fibrotic ILD, scar tissue develops within the lung tissue, making the lungs stiffer and less able to expand easily. As a result, breathing requires more effort. At the same time, fibrosis can impair the transfer of oxygen from the lungs into the bloodstream, particularly during physical activity. In practical terms, this means the lungs may not fill as easily, oxygen levels may fall more quickly with exertion, and the body compensates by increasing breathing rate. Breathlessness is therefore often most noticeable when walking uphill, climbing stairs, or carrying loads. In earlier stages of disease, patients may feel entirely comfortable at rest but notice reduced stamina during activity. As disease progresses, symptoms may occur at lower levels of exertion.
Why can breathlessness feel worse than the tests suggest?
Many patients are surprised that their level of breathlessness does not always match their lung function results. This is because breathlessness is a complex physiological and perceptual experience. Reduced physical activity over time can lead to loss of muscle strength and cardiovascular fitness (deconditioning) which increases the effort required for everyday tasks. Anxiety can also amplify breathlessness; anticipating shortness of breath may heighten the sensation itself. In some patients, pulmonary hypertension (raised pressure in the lung circulation) contributes to exertional limitation. Anaemia, heart disease, acid reflux, or sleep disturbance may further compound symptoms.
Oxygen saturation levels and lung function tests measure physiological change, but they do not always correlate perfectly with symptom intensity. The brain interprets breathing effort through multiple sensory pathways. This explains why two patients with similar lung function may experience breathlessness differently. For this reason, breathlessness often reflects the interaction between lung mechanics, oxygen transfer, cardiovascular response, physical conditioning, and emotional state. It is rarely driven by a single factor.
Although breathlessness can feel alarming, particularly during exertion, it is not usually a sign that the body is “running out of air.” The sensation typically reflects increased breathing effort rather than immediate danger. Understanding this distinction can reduce fear and help break the cycle between anxiety and breathlessness.
Does worsening breathlessness always mean the disease is progressing?
Not necessarily. Breathlessness can fluctuate from day to day depending on fatigue, stress, physical activity, or minor inter-current illness. A temporary increase in symptoms does not automatically indicate fibrosis is worsening. True disease progression is assessed through structured monitoring over time, including trends in lung function, repeat imaging, and sustained symptom evolution. Single fluctuations are common and require context. However, steadily worsening breathlessness, particularly if accompanied by reduced exercise tolerance or falling oxygen levels, should prompt clinical review to exclude infection, fluid retention, pulmonary hypertension, or genuine progression.
What can help breathlessness?
Although established lung scarring cannot currently be reversed, breathlessness can often be improved or better managed. Optimising treatment of the underlying ILD is the first step to help stabilise disease behaviour. Pulmonary rehabilitation is one of the most effective interventions available. Structured exercise training improves muscle efficiency, reduces oxygen demand during exertion, and builds confidence. Even when lung function does not change, patients frequently report meaningful improvement in exercise capacity and quality of life. Avoiding activity because of breathlessness can unintentionally worsen symptoms, and carefully supervised reconditioning is protective.
Oxygen therapy may be beneficial in patients who experience oxygen desaturation during exertion, even if resting levels are normal. Ambulatory oxygen can improve exercise tolerance and reduce strain in selected individuals. Its prescription is guided by formal assessment and tailored to need. Requiring oxygen does not automatically indicate advanced disease.
Simple breathing strategies can also reduce symptom intensity. Techniques such as paced breathing, pursed-lip breathing, and activity pacing help control the sensation of air hunger. Specialist respiratory physiotherapists can provide individualised guidance.
Equally important is identifying and treating contributing conditions. Anaemia, cardiac disease, acid reflux, sleep-disordered breathing, and anxiety can all worsen breathlessness. Addressing these factors may significantly improve symptoms even when fibrosis itself remains unchanged.
Is severe breathlessness inevitable?
Breathlessness exists on a spectrum. Some patients remain mildly limited for many years. Others experience gradual progression. The rate of change varies significantly between individuals, and structured monitoring allows clinicians to detect meaningful shifts early.
Many patients adapt remarkably well. Adjusting pacing, building physical resilience, and using supportive therapies can maintain independence and quality of life even in the presence of fibrosis. The experience of breathlessness often becomes more manageable once its mechanisms are understood and strategies are in place.
When should specialist review be sought?
Specialist ILD review is particularly important if breathlessness is worsening more rapidly than expected, if oxygen levels appear to be falling, if previously manageable activities become difficult, or if there is concern about pulmonary hypertension or other contributing conditions. Because breathlessness can arise from multiple overlapping causes, careful reassessment of lung function, imaging, oxygen saturation, and cardiac status may be required to clarify the driver of symptoms. Small differences in interpretation can meaningfully influence management decisions.
